1099 B with “Final principal payment” what to do with

Ok, I have a 1099B with three transactions listed, none of the have a
Date of Acquisition, cost or other basis, or Gain or loss

The first one has a note that says “principal payment” and gives a cost basis factor

The second two have a note that says “Final principal payment”

I am not sure how to report this on Sch. D

Since this is a return of principal do I need the cost basis and date acquired?

Just put the cost and sales price the same for no gain or loss if this is truly a "return of principle"
