Format of Tuition for tax

What is the template of tuition receipt (for tax purpose/reimbursement) to give to a customer in prek school?
Please which format is correct?
1. Received $3,000 in three payments for X son of Y , date of birth m/d/y, for K1 program service for the school year 2012-2013. Below is the detail of the payments:
First payment as deposit of $1,000 received on 5/31/12
Second payment of $1,000 received on 8/1/12
Third payment of $1,000 received on 12/1/12".

2. Received $1,000 dated: 05/31/12 for X daughter Y, date of birth m/d/y, for K1 program service dated from 06/30/12 to 07/31/12.
Received $,000 dated: 08/01/2012 for X daughter of Y, date of birth m/d/y, for K1 program service dated from 08/02/12 to 11/30/12.
Received $1,000 dated: 12/01/12 for X daughter of Y, date of birth m/d/y, for K1 program service dated from 12/2/12 to 12/31/12

Is it based on payment date (school year) or service? thank you

First off I would not include date of birth even if you were given the certified birth records. Certainly, the name of the child but again not the relationship to the adult. What the parents need (besides your tax id which you can give them on a w-7) is the date the service was performed, the type of service (pre k) , and the amount and date of payment. Double check this with your tax adviser.
So the answer to your question is both date of service and date of payment are needed by the parent.
