UK Self employed deliver driver SA302

Hi, I am doing my SA302 I am a sole trader joiner now but for a couple of months I have been a self employed delivery driver. I was being paid £100 per day, reimbursed for mileage @ £0.1426 per mile, and I was charged £185 per week for van hire + van insurance. I don't know how to add the income and the expenses from that period onto the SA302.
This is an example of an invoice, can you kindly help by advising what should I put as:
a) income: I was paid £100 per day x 5 days = £500; should I use this figure as the income or should I use the actual figure paid by the client, after all additions (mileage paid) and deductions (van hire&insurance), which was £388.44?
b) the mileage reimbursed as expenses-do I add it as expense in the SA302? do I deduct the amount paid to me aready, £0.1426 per mile?
c) the £185 deducted by client for van hire, VAT & insurance-do I add this to the SA302 and claim as expense?

Invoice:
total miles: 515 x £0.1426 = £73.44
total routes 5 x £100 = £500
total deductions: -£185

total: £388.44
