USA HSA distribution

So for 2014, I have an HSA account because of the high deductable insurance through my employer.

I refilled a prescription at a pharmacy and charged it to my HSA account, via debit card. Now, I get a tax form saying that transaction is taxable HSA Distribution.

I always thought that medications filled at the pharmacy tax-free for HSA?
