USA Charity is personal account

good evening everyone,

I need to transfer pretty huge charity amount to a friend’s personal account for brain surgery of a 3years old. What forms/measures should I take to report/justify this amount to IRS during tax filing,

Unless they are a qualified 501c3 charity, there's not much you need to do unless there is an individual gift over the gift tax limit.

You can probably arrange to pay this hospital directly, as I'm sure the hospital or its foundation is a 501c3 that would be able to accommodate the transaction.
