USA Completing W-9

We have hired an individual. Instead of doing the W4/W2 IRS option we elected to do the W9/1099-NEC. On Question #3 - they are an individual hourly employee, total of 6hrs per month, no LLC. I am thinking that the Individual/sole proprietor or single member LLC is the correct box to check, correct? I see the Other box but not sure if this is an option and what would be other instructions?

#2 - they do not have a business name so this would be left blank, correct?
#4 - Exemptions, nothing entered here, correct?

What would be entered is the #1 - Name of Individual and the #5,#6 for address.
#7 and Request name is blank

Part 1 would be completed with individual's SSN.
They would sign and date.

I am very familiar with the W4/W2 process but this is the first time for W9 and then the 1099-NEC.

The idea of the form is that you, as employer, just hand it to the worker. The worker then fills it out, signs it, and hands it back to you. The worker, not you, knows what their own tax situation is. I know that does not sound helpful to the worker. But you might have them read this
